ABSTRACT

Introducción: En las últimas décadas la incidencia del cáncer tiroideo en el curso de la enfermedad nodular se ha incrementado debido a las novedosas técnicas de diagnóstico; sin embargo, la tasa de mortalidad se ha mantenido muy baja. Objetivo: Evaluar las características clínicas, epidemiológicas y quirúrgicas de pacientes con afecciones nodulares tiroideas. Métodos: Se realizó un estudio descriptivo observacional de cohorte prospectivo, longitudinal con los pacientes operados de afecciones tiroideas durante el período comprendido entre enero del 2008 y diciembre del 2018. El universo y la muestra quedaron constituidos por 467 pacientes que cumplieron con los criterios de inclusión. Resultados: Predominaron el sexo femenino (89,5 por ciento) y el grupo etario de 45-60 años (29,5 por ciento). Asociaron comorbilidades 338 pacientes y algún factor de riesgo de malignidad (6,2 por ciento). Un total de 174 pacientes manifestaron síntomas y 264 mostraron algún signo. Predominaron los reportes ecográficos (TI-RADS) y citológicos (Bethesda) tipo II (54,3 por ciento) y (55,5 por ciento), respectivamente. La hemitiroidectomía fue el procedimiento más realizado (59,9 por ciento) y la disfonía la complicación más encontrada (1,9 por ciento). Conclusiones: El diagnóstico oportuno del cáncer tiroideo en el curso de una enfermedad nodular contribuye a individualizar todas las decisiones terapéuticas atendiendo a las características de cada paciente y sus circunstancias(AU)


Introduction: In recent decades, the incidence rates of thyroid cancer in the course of nodular disease has increased due to novel diagnostic techniques; however, the mortality rate has remained very low. Objective: To evaluate the clinical, epidemiological and surgical characteristics of patients with nodular thyroid disease. Methods: A descriptive, observational, of prospective cohort, longitudinal and observational study was conducted with patients operated on for thyroid disorders during the period from January 2008 to December 2018. The study universe and sample consisted of 467 patients who met the inclusion criteria. Results: The female sex (89.5 percent) and the age group 45-60 years (29.5 percent) predominated. Comorbidities were present in 338 patients, as well as some risk factor for malignancy in 6.2 percent. A total of 174 patients manifested symptoms and 264 showed some sign. There was a predominance of echography (TI-RADS) and cytology (Bethesda) type II reports, accounting for 54.3 percent and 55.5 percent, respectively. Hemithyroidectomy was the most performed procedure (59.9 percent), while dysphonia was the most encountered complication (1.9 percent). Conclusions: Timely diagnosis of thyroid cancer in the course of nodular disease contributes to individualizing all therapeutic decisions considering the characteristics of each patient and their circumstances(AU)

ABSTRACT

Objective:To investigate the risk factors of positive peritoneal cytology (PPC) in patients with endometrial cancer and the impact of PPC on patients' prognosis.Methods:The clinicopathological data of 202 patients who underwent initial surgical treatment and were diagnosed with endometrial cancer by postoperative pathology at Qilu Hospital of Shandong University from January 2015 to December 2019 were retrospectively analyzed, and the peritoneal fluid of patients were sent intraoperatively for cytological liquid-based smear examination. Logistic regression was used to perform univariate and multivariate analyses of PPC in the whole group of patients and the early-stage patients; Univariate analysis of the progression-free survival in the whole group of patients and the early-stage patients was performed by Kaplan-Meier method and compared by log-rank method, and multivariate analysis of the progression-free survival in the whole group of patients and the early-stage patients was performed by Cox proportional hazards model.Results:Of 202 patients, 183 (90.6%) had negative peritoneal cytology (NPC) and 19 (9.4%) had PPC; 180 patients (89.1%) were stage Ⅰ-Ⅱ and 22 (10.9%) were stage Ⅲ-Ⅳ; 180 patients (89.1%) had early-stage endometrial cancer. Deep myometrial infiltration ( OR = 3.57, 95% CI 1.02-12.45, P = 0.046) and lymph node metastasis ( OR = 7.16, 95% CI 1.70-30.23, P = 0.007) were independent risk factors for PPC in patients with endometrial cancer; deep myometrial infiltration was an independent risk factor for PPC in patients with early-stage endometrial cancer ( OR = 6.22, 95% CI 1.22-31.73, P = 0.028). The 3-year PFS rates for the whole group of patients with PPC and NPC were 72.9% and 92.7%, and the difference was statistically significant ( P = 0.001); the 3-year PFS rates for early-stage patients with PPC and NPC were 82.5% and 96.2%, and the difference was statistically significant ( P = 0.002). PPC was an independent risk factor for PFS in the whole group of patients with endometrial cancer ( HR = 4.80, 95% CI 1.14-20.17, P=0.032); PPC was also an independent risk factor for PFS in patients with early-stage endometrial cancer ( HR = 8.85, 95% CI 1.96-39.93, P = 0.005). Conclusions:Deep myometrial infiltration is an independent risk factor for PPC, and PPC is an independent risk factor for PFS in patients with endometrial cancer. Routine cytological examination of peritoneal fluid is recommended in patients with endometrial cancer.

ABSTRACT

Objective:To evaluate the significance of copy number variation (CNV) and metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) in the diagnosis of meningeal carcinomatosis (MC).Methods:Ten patients with MC diagnosed in the Department of Neurology of Peking Union Medical College Hospital from March 2022 to June 2022 were consecutively enrolled in this study. The patients were diagnosed according to the criteria of the Chinese expert consensus on the diagnosis of MC by the Chinese Society of Infectious Diseases and Cerebrospinal Fluid Cytology, and the diagnosis of MC was confirmed by CSF cytology. The control group included 10 patients who were diagnosed as autoimmune encephalitis or viral encephalitis. CSF mNGS and CNV analysis were performed simultaneously in all the patients.Results:Of the 10 patients with MC, 6 had lung adenocarcinoma, 4 had breast cancer. CSF mNGS and CNV analysis detected large CNV in 8 of 10 patients with MC, including 4 patients with breast cancer and 4 patients with lung cancer. The results of pathogenic microorganism analysis of CSF mNGS in all the patients were negative. Meanwhile, large CNV was not detected in the control group.Conclusions:CSF CNV can serve as a diagnostic marker for MC. The combination of mNGS and CNV analysis has demonstrated a high positive rate in the diagnosis of MC. The dual-omics analysis of pathogenic microorganisms and CNV has been proposed as a potential strategy to further expand the clinical utility of CSF mNGS in the realm of auxiliary diagnosis.

Introduction: Cytopathology is a collection method that allows cell analysis through the different techniques. The oral mucosa exfoliated cells observation demonstrates morphological, biochemical and/or molecular aspects depending on the type of processing of the sample. Aim: This study tested the use of oral cytopathology associated with transmission electron microscopy (TEM) to observe the morphology of cells, mainly in relation to the cell nucleus, the cytoplasmic membrane, and cell junctions. Materials and Methods: Exfoliated epithelial cells from the oral mucosa were analyzed by TEM from individuals exposed to tobacco and alcohol, with leukoplakia or with a histopathological diagnosis of squamous cell carci-noma. Results: The cytoplasmic cell-cell junctions in the malignant samples lost the characteristic irregular pattern formed by the numerous interdigitations and the junctional process of normal cells and started to present a straight cytoplasmic surface. The nuclei of cells from leukoplakia and squamous cell carcinoma samples showed heterogeneous staining, while non-lesional cells were homogeneous. Discussion: The analysis of oral cytopathological smears by TEM contributes to the un-derstanding of the changes that occur during the process of malignancy of the oral mucosa, especially with regard to the cytoplasmic membrane and intercellular junctions. Conclusion: TEM may be a good analytical method to investigate morphological changes in exfoliated cells of the oral epithelium.


Introdução: A citopatologia é um método de coleta que permite a análise celular por meio de diferentes técnicas. A observação das células esfoliadas da mucosa bucal demonstra aspectos morfológicos, bioquímicos e/ou moleculares dependendo do tipo do processamento empregado. Objetivo: Este estudo testou o emprego da técnica de citopatologia bucal associada à microscopia eletrônica de transmissão (MET) para observar a morfologia das células, principalmente com relação à membrana citoplasmática, as junções celulares e ao núcleo da célula. Materiais eMétodos: Células epiteliais esfoliadas da mucosa bucal foram analisadas por MET de indivíduos expostos a tabaco e álcool, apresentando leucoplasia ou com diagnóstico de carcinoma espinocelular. Resultados:As junções citoplasmáticas célula-célula nas amostras malignas perderam o padrão irregular característico formado pelas inúmeras interdigitações e o processo juncional das células normais e passaram a apresentar uma superfície citoplasmática reta. O núcleo das células das amostras de leucoplasia e do carcinoma espinocelular apresentou coloração heterogênea, enquanto as células não lesionais foram homogêneas. Discussão: A análise de esfregaços citopatológicos bucais por MET contribui para o entendimento das alterações que ocorrem durante o processo de malignidade da mucosa bucal, principal-mente no que diz respeito à membrana citoplasmática e as junções intercelulares. Conclusão: A MET pode ser um bom método analítico para investigar alterações morfológicas em células esfoliadas do epitélio bucal.

ABSTRACT

Introducción: Desde los inicios de este siglo se ha producido un notable incremento mundial de la tasa de incidencia del cáncer de tiroides, el cual generalmente tiene un curso larvado y asintomático. Objetivo: Profundizar en el conocimiento de los aspectos novedosos del diagnóstico oportuno y tratamiento personalizado del cáncer tiroideo. Desarrollo: El cáncer tiroideo es la enfermedad maligna más frecuente del sistema endocrino. En las últimas décadas, su incidencia se ha incrementado aceleradamente, aunque la mortalidad se ha mantenido baja. El descubrimiento y desarrollo de nuevas técnicas de imágenes, inmunológicas y moleculares, han permitido estudiar en profundidad la neoplasia de la tiroides. Esto ha favorecido avanzar en los aspectos que más han modificado la nueva actitud respecto al diagnóstico oportuno y su tratamiento. Conclusiones: En años recientes, los avances de las investigaciones básicas, clínicas y traslacionales (aplicación real de los conocimientos básicos en la práctica clínica), han transformado antiguos conceptos relacionados con el cáncer tiroideo y han dotado de nuevas herramientas para el diagnóstico oportuno y tratamiento personalizado.


Introduction: Since the beginning of this century there has been a notable increase worldwide in the incidence rate of thyroid cancer, which generally has a latent and asymptomatic course. Objectives: To deepen the knowledge of the novel aspects of timely diagnosis and treatment of thyroid cancer. Development: Thyroid cancer is the most frequent malignant disease of the endocrine system. In recent decades, its incidence has increased rapidly, although mortality has remained low. The discovery and development of new imaging, immunological and molecular techniques have made it possible to study thyroid neoplasm in depth. This has favored advancing in the aspects that have most modified the new attitude regarding timely diagnosis and its treatment. Conclusions: In recent years, advances in basic, clinical and translational research have transformed old concepts related to thyroid cancer and have equipped with new tools for timely diagnosis and personalized treatment.

Introducción. La detección del virus del papiloma humano mediante la combinación de la prueba de HPV y otras técnicas como la citología, ha demostrado su eficacia en el diagnóstico y tratamiento oportuno de lesiones asociadas con el cáncer de cuello uterino. Objetivo. Estimar el impacto presupuestal de la estrategia de detección temprana del HPV mediante la prueba de genotipificación combinada con la citología en comparación con la citología convencional, en mujeres de 30 a 65 años participantes en el programa de tamizaje de cáncer de cuello uterino en una Entidad Administradora del Plan de Beneficios en salud (EAPB) en Colombia. Materiales y métodos. Utilizando un árbol de decisiones y un modelo de Markov, se estimaron las implicaciones clínicas y los costos directos anuales de dos ciclos de tamizaje, diagnóstico y tratamiento, en una cohorte de mujeres. Las prevalencias de los resultados clínicos y los costos se tomaron de la base de datos de una EAPB y la información de la progresión, persistencia y regresión de los estados de salud provinieron del estudio ATHENA. Resultados. El esquema de tamizaje con la prueba de HPV, la genotipificación y la citología resultó en un ahorro de costos comparado con la citología convencional. El costo promedio por ciclo de tamizaje con la prueba de HPV se estimó en COP $129'201.363 y con la citología en COP $186'309.952, es decir, un ahorro de COP $57'108.589 (30,7 %). Conclusión. La implementación de la estrategia de tamizaje evaluada sugiere que habría ahorros derivados de la detección temprana de los estados de salud asociados con el desarrollo de cáncer de cuello uterino.


Introduction: The detection of the human papillomavirus (HPV) through the combination of the HPV test and other techniques such as cytology has impacted the detection and timely treatment of lesions associated with cervical cancer. Objective: To estimate the budgetary impact of the strategy of early detection of HPV with DNA test genotyping with reflex cytology versus conventional cytology in women aged 30 to 65 years attending the cervical cancer screening program at a health benefit managing entity in Colombia. Materials and methods: Using a decision tree and a Markov model, the clinical implications and direct costs of screening, diagnosis, and treatment were estimated in a cohort of women. The analysis considered two screening cycles and their annual costs. The data on the prevalence of clinical results and the costs were taken from the health managing entity. The information on the progression, persistence, and regression of the health states were taken from the ATHENA study. Results: The screening scheme with the HPV test, genotyping, and reflex cytology compared to conventional cytology was cost-saving. The average cost per screening cycle with the HPV test was estimated at COP $ 129,201,363 and with cytology at COP $ 186,309,952, i.e., a saving of COP $ 57,108,589 (30.7%). Conclusion: The implementation of the screening strategy under evaluation suggests prospective savings derived from the early detection of health states associated with the development of cervical cancer.

Resumen OBJETIVO: Determinar la prevalencia de lesiones intraepiteliales de alto grado en mujeres de 15 a 25 años, con un reporte de citología de cambios en el epitelio atribuibles a la infección por el virus del papiloma humano. MATERIALES Y MÉTODOS: Estudio retrospectivo, transversal y descriptivo efectuado con base en la información de los expedientes de mujeres de 15 a 25 años atendidas entre los meses de enero del 2015 a diciembre del 2020 en la Clínica de Displasias del Hospital General Regional 1 de Querétaro, México, con reporte de citología con cambios en el epitelio atribuibles a la infección por el virus de papiloma humano.Criterios de inclusión: Expedientes de mujeres de 15 a 25 años con reporte de cambios en el epitelio atribuibles a la infección por el VPH reportados en la citología. Criterio de exclusión: Expediente incompleto. El plan de análisis estadístico incluyó: promedios, intervalos de confianza para promedios, porcentajes e intervalo de confianza para porcentajes. RESULTADOS: Se revisaron 823 expedientes con reporte de citología con cambios en el epitelio atribuibles a la infección por VPH. La edad promedio de las mujeres fue de 23.28 años (IC95%: 23.24-23.52), la edad media al inicio de la vida sexual fue de 16.01 años (IC95%: 15.92-16.10). El 7.0% (IC95%: 5.3-8.7) tuvieron lesiones intraepiteliales de alto grado en la citología y en la colposcopia. CONCLUSIÓN: La prevalencia de lesiones intraepiteliales de alto grado en mujeres de 15 a 25 años con reporte de cambios en el epitelio atribuibles a la infección por el VPH es del 7.0% tanto para la citología como para la colposcopia.


Abstract OBJECTIVE: To know the prevalence of high-grade intraepithelial lesions in women aged 15 to 25 years, with a cytology report of changes in the epithelium attributable to human papillomavirus infection. MATERIALS AND METHODS: Retrospective, tranversal and descriptive study performed at the Dysplasia Clinic of the Regional General Hospital 1 of Querétaro, Mexico, from January 2015 to December 2020. Inclusion criteria: files of women aged 15 to 25 years with report of changes in the epithelium attributable to HPV infection reported on cytology. Exclusion criteria: incomplete records. The statistical analysis plan included: averages, confidence intervals for averages, percentages, and confidence interval for percentages. RESULTS: We reviewed 823 files with cytology reports of epithelial changes attributable to HPV infection. The mean age of the women was 23.28 years (95%CI: 23.24-23.52), the mean age at sexual debut was 16.01 years (95%CI: 15.92-16.10). 7.0% (95%CI: 5.3-8.7) had high-grade intraepithelial lesions on cytology and colposcopy. CONCLUSION: The prevalence of high-grade intraepithelial lesions in women aged 15-25 years with reported epithelial changes attributable to HPV infection is 7.0% for cytology and for colposcopy.

ABSTRACT Objective: To obtain data about the evaluation of thyroid nodules (TNs) in the northeastern of the State of São Paulo, compared by health care type, and measure the performance of cytology as a screening test for thyroid cancer (TC). Subjects and methods: We collected data of 597 patients treated in the Brazilian public health care system (SUS), supplementary health (SH) and in private health system (PHS) in 2014. A total of 803 TNs were aspirated, and 125 patients underwent surgery. Results: The distribution of all cytologic results according to the Bethesda system was: I, 135 (16.8%); II, 475 (59.2%); III, 107 (13.3%); IV, 32 (4.0%); V, 20 (2.5%); VI, 34 (4.2%). The time between cytologic analysis and surgery was longer in the SUS than in the SH for TNs in general (p < 0.001) and for TNs with Bethesda V and VI cytology (p = 0.01). The sizes of the TNs and resected malignant TNs was larger in the SUS than in the SH (p = 0.001 and p = 0.02, respectively). The number of PHS surgeries was too small and was not compared. The prevalence of TC was 9.2% and 23.6% of them were treated in the SUS. Cytology showed a 93.6% sensitivity, 95.8% specificity, and 94.7% accuracy when Bethesda III and IV were excluded. Conclusion: Cytology was a good screening test for TC categories Bethesda II, V, and VI. The differences between the SUS and SH indicate a need for improved access to consultations and specialized tests in the SUS.

ABSTRACT

Objective: To qualitatively investigate whether a prototype brush composed of metal bristles collects oral epithelial cells effectively for cytological evaluation of oral mucosal lesions. Material and Methods: Twenty patients with suspicious oral mucosal lesions were enrolled. Patients were asked to gargle with saline and to deposit the oral rinse into specimen cup. Then, oral mucosal cell samples were collected using a metal oral brush, via sweeping motion. Punch biopsy was performed for histological examination. All samples were evaluated with liquid based cytology (LBC) according to the cellularity, the depth of the epithelial layer, cellular integrity by an oral pathologist. Results: Oral rinse provided samples with 100% cellular integrity and cellularity, mostly from the intermediary layers. With metal brush, both inadequate cellularity and cellular integrity was observed in 25% of the cases. Cellular integrity was adequate in 65%, cellularity was adequate in 45% of the lesions. Samples were dominantly from the intermediary layers, but in one case, metal brush collected cells from the parabasal layer. Conclusion: The narrow spiral pitch and width of metal bristles may have resisted to release the cellular samples collected. With adjustment of the spiral pitch and diameter of metal brush bristles, its' efficacy could be enhanced.

ABSTRACT

PURPOSE: Fine-needle aspiration biopsy (FNAB) can be used to diagnose thyroid cancer and other tumors. Although FNAB without negative pressure (FNAB−P) reduces the risk of blood contamination, FNAB with negative pressure (FNAB+P) increases the sensitivity of the biopsy results. Therefore, we performed a randomized study of FNAB with or without negative pressure to identify the better diagnostic method. METHODS: Between March 2016 and February 2017, 172 consecutive patients were enrolled to investigate >0.5 cm nodules with indeterminate or suspicious malignant features. Patients were randomly assigned to the FNAB+P group (a 50 mL syringe was used to provide negative pressure) or to the FNAB−P group (passive collection of blood in the needle's hub). The 2 methods' diagnostic adequacy and quality were evaluated using an objective scoring system. The study's protocol was registered with the World Health Organization Clinical Research Information Service (http://cris.nih.go.kr/cris, KCT0001857). RESULTS: The patients were randomly assigned to the FNAB+P group (n = 86) or the FNAB−P group (n = 86). There were no significant intergroup differences in nodule position, size, age, consistency, calcification, BRAF mutation, or pathology. Evaluation of diagnostic adequacy parameters revealed no significant differences in background blood/clot (P = 0.728), amount of cellular material (P = 0.052), degree of cellular degeneration (P = 0.622), degree of cellular trauma (P = 0.979), or retention of appropriate architecture (P = 0.487). Furthermore, there was no significant intergroup difference in the diagnostic quality (P = 0.634). CONCLUSION: This prospective randomized study failed to detect significant differences in the diagnostic adequacy and quality of FNAB with or without negative pressure. Therefore, the examiner may select whichever FNAB method they prefer.

ABSTRACT

This article focuses on the controversial issues in the diagnosis and treatment of thyroid cancer. Several commonly used molecular tests, especially gene mutation and fusion detection, in the preoperative diagnosis of thyroid nodules and the prognosis evaluation of thyroid cancer are introduced. For the thyroid nodules with uncertain indeterminate cytology, different molecular screening methods have their own advantages and disadvantages. Among them, DNA detection, especially multi-gene molecular testing based on next-generation sequencing has better sensitivity and specificity. Besides being used in diagnosis, it can also detect gene mutations related to tumor prognosis, such as BRAFV600E mutation and TERT promoter mutation, which is increasingly widespread and accepted.

Objective@#To investigate the diagnostic performance of fine-needle aspiration (FNA) cytology for the detection of lateral lymph node metastases in patients with papillary thyroid carcinoma (PTC).@*Methods@#A total of 109 lateral lymph nodes with suspicious metastases from 85 patients were retrospectively subjected to FNAC, fine-needle aspiration thyroglobulin measurement (FNATg), and FNATg/SerumTg measurement. Lymph node pathological results after surgery were taken as the gold standard. Using Mann-Whitney U test, Pearson linear model and ROC curve were used for data analysis.@*Results@#The sensitivity, specificity and accuracy of FNATg for the diagnosis of lateral neck lymph node metastasis were respectively 93.7%, 90.0% and 93.3% and those of FNATg/SerumTg were respectively 89.9%, 90.0% and 93.2% respectively, the threshold values for FNATg and FNATg/SerumTg were 0.925 ng/ml and 1.007, respectively. The sensitivity, specificity and accuracy of FNATg combined with FNAC were respectively 91.0%,93.5% and 94.4%. The existence of thyroid tissue and the expression of serum Tg did not affect the expression of lymph node FNATg. The FNATg cutoff value of 0.925 ng/ml showed the best diagnostic performance in patients with a thyroid gland, while the FNATg/SerumTg cutoff ratio of 14.95 showed the best diagnostic performance in patients without a thyroid gland. The serum TgAb significantly interfered with the expression of FNATg in the lateral neck metastatic lymph nodes (P=0.049).@*Conclusions@#FNATg alone or the combination of FNATg with FNAC are highly reliable in the diagnosis of lateral neck lymph node metastases in patients with PTC. The expression of TgAb may interfere with the accuracy of the diagnostic performance of FNATg.

ABSTRACT

Objective@#To evaluate the utility of the BRAFV600E mutation detection in different cytoloy categories and to determine if the VE1 antibody can serve as a screening tool for the detection of BRAFV600E mutation in thyroid fine needle aspiration (FNA) specimens.@*Methods@#A total of 273 FNA residual specimens were collected. BRAFV600E testing was performed on these liquid-based specimens. And also 78 specimens with enough residual cells were stained with VE1 antibody. Comparisons of molecular and immunocytochemistry results with clinicopathological outcomes were performed. SPSS 17.0 software was used to analyze the data.@*Results@#There were 70 indeterminated diagnoses in 273 cases with FNAs. Fifty-eight cases were proven to be papillary thyroid carcinoma (PTC) by histology, including 9 cases of atypia of undetermined significance/follicular lesion of undetermined significance (AUS/FLUS), 3 cases of follicular neoplasm/suspicious for follicular neoplasm (FN/SFN), and 46 cases of suspicious for malignancy (SM). BRAFV600E analysis detected PTC in 3 of 9 cases with AUS/FLUS, and in 31 (67.4%) of 46 cases with SM. The sensitivity of immunostaining with VE1 antibody was 62.8%(27/43) and the specificity was 91.4% (32/35). VE1 expression showed moderately concordance with the molecular mutation (κ=0.524, P<0.001).@*Conclusions@#Additional BRAFV600E detecting can improve the diagnostic efficacy for PTC in AUS/FLUS and SM. VE1 expression may be an alternative method for BRAFV600E detecting when molecular detecting is unavailable.

ABSTRACT

Objective To investigate the dynamic changes of cerebrospinal fluid (CSF) cytology in the course of tuberculous meningitis (TBM) and its diagnostic value.Methods 37 children with TBM who were treated in the department during January 2008 to December 2016 were selected as the observation group.Another 40 children with non-TBM (including viral encephalitis and suppurative encephalitis) were selected as the control group at the same time.The CSF samples were collected for routine,biochemical and CSF cytological examinations.The detection of mycobacterium tuberculosis antigen pure protein derivative (PPD) positive cell rate (monocyte) was performed,and the value of CSF cytology in the diagnosis of TBM was summarized.Results (1) The lumbar puncture pressure,white blood cell count,lymphocyte count and protein quantification in the observation group were higher than those in subgroups of the control group,while the glucose and chloride levels were lower than those in the subgroups (P < 0.05).(2) The cytological examination of CSF in the observation group mainly was mixed cell reaction,accounting for 83.78%,followed by lymphocyte reaction (10.81%).Viral meningitis mainly manifested as lymphocyte reaction,accounting for 82.14%,while suppurative meningitis mainly manifested as polymorphonnclear response,accounting for 75.00%.(3) The positive rate of PPD antigen in the observation group was 81.08%,which was significantly higher than that in children with viral meningitis,suppurative meningitis (P < 0.05).(4) The accuracy rate of protein level >0.45 g/L in the diagnosis of TBM was 89.19%,and the accuracy rate of CSF cytological mixed cell response was 83.78%.(5) The white blood cell count,the proportion of monocytes and the proportion of neutrophils were the highest in children with tuberculous meningitis less than 2 weeks,compared with those in children with 3-4 weeks and >4 weeks (P <0.05).The above-mentioned cytological indexes were decreased while the proportion of lymphocytes was increased in children with TBM whose course of disease was 3-4 weeks.The white cell count,proportions of monocytes,lymphocytes and proportion of neutrophils in children whose course of disease was longer than 4 weeks were lower than those in children whose course of disease was 3-4 weeks (P < 0.05).Conclusions The white blood cell count,proportion of monocyte and proportion of neutrophils were relatively higher in the first 2 weeks of TBM,and maintained for 2 weeks.2 weeks later,levels of above-mentioned cells decreased except for lymphocytes while the proportion of lymphocytes increased continuously and then decreased gradually 4 weeks later.The diagnostic accuracy rate of CSF cytological mixed reaction to TBM was relatively higher,and the consistency with tuberculosis antigen PPD examination was high,which could be used as a choice for early diagnosis of TBM.

Introduction: The anal lesions seem to have a natural history that closely resembles cervical lesions, with signs that precede the invasion. Cytological changes of anal epithelium induced by HPV can be detected through cytology, as it is considered an effective screening method. Objective: To identify the frequency of atypical epithelial conventional cytology results by comparing anal samples through Liqui-PREPTM technology in HIV-positive men. Methods: Cross-sectional descriptive and analytical study of 33 men who have sex with men (MSM), HIV-positive and anoreceptive attended at the Gaffrèe and Guinle University Hospital (HUGG), Rio de Janeiro, from June to July, 2016. Collection of anal samples for the conventional cytology and Liqui-PREPTM cytology was carried out. For significance of findings, Fisher exact test with 95% confidence interval was used and cytological Kappa index was employed for concordance between the two cytological methods. Results: The age ranged from 23 to 60 years (mean=39.06). The CD4 cell count was between 200 to 500/mm3 on 16 (48.5%) and 13 (39.4%), and 50% was diagnosed with HIV for more than 6 years. In conventional cytology one case was considered unsatisfactory (3%). Among the cases considered satisfactory, 9 (28.1%) were diagnosed with ASC-US; 4 (12.5%) LSIL; 2 (6.3%) ASC-H, and 2 (6.3%) HSIL. Through Liqui-PREPTM method, 7 cases were considered unsatisfactory (21.2%). Among the satisfactory cases, 7 showed ASC-US (26.9%); 4 (15.4%) ASC-H; 2 (7.7%) LSIL; and 2 (7.7%) HSIL. The difference of unsatisfactory cases between both methods, although higher for Liqui-PREPTM, was not statistically significant (p=0.054). The correlation was moderate (0503; p<0.006 [0.1765­0.8298]). Conclusion: The cytologic atypia is common among MSM HIV (+), and the anal conventional cytology and liquid by Liqui-PREPTM cytology are equivalent, although they are more unsatisfactory in the latter technique.

As alterações citológicas do epitélio anal induzidas pelo HPV podem ser detectadas por citologia, um método de rastreio considerado efetivo. Objetivo: Identificar a frequência de atipias epiteliais nos resultados da citologia convencional comparando amostras anais pela tecnologia Liqui-PREP® em homens HIV positivos. Métodos: Estudo transversal, descritivo e analítico de 33 homens que fazem sexo com homens (HSH), HIV positivos e anorreceptivos atendidos no Hospital Universitário Gaffrèe e Guinle (HUGG), Rio de Janeiro, no período de junho a julho de 2016. Os pacientes foram submetidos à coleta de amostras anais para citologia convencional e citologia Liqui-PREP®. Para significância de achados, foi usado o teste exato de Fisher com intervalo de confiança de 95%, e para concordância entre os dois métodos citológicos, foi utilizado o índice de Kappa. Resultados: A idade variou de 23 a 60 anos (média=39,06). A contagem de células CD4 foi entre 200 e 500/mm3 para 16 (48,5%) e 13 (39,4%) dos casos analisados, e 50% tinham o diagnóstico de HIV há mais de seis anos. Na citologia convencional, um caso foi considerado insatisfatório (3%). Entre os casos considerados satisfatórios, 9 (28,1%) foram diagnosticados como células escamosas atípicas de significado indeterminado possivelmente não neoplásicas (ASC-US); 4 (12,5%) como lesão intraepitelial de baixo grau (LSIL); 2 (6,3%) como células escamosas atípicas não sendo possível excluir lesão intraepitelial de alto grau (ASC-H) e 2 (6,3%) como lesão intraepitelial de alto grau (HSIL). Pelo método Liqui-PREP®, 7 casos foram considerados insatisfatórios (21,2%). Entre os casos satisfatórios, 7 como ASC-US (26,9%); 4 (15,4%) como ASC-H; 2 (7,7%) como LSIL e 2 (7,7%) como HSIL. A diferença de insatisfatório entre os métodos, embora maior para Liqui-PREP®, não foi estatisticamente significativa (p=0,054). A concordância foi moderada (0,503; p<0,006 [0,1765­0,8298]). Conclusão: É frequente a atipia citológica entre HSH HIV (+), e as citologias anal convencional e em meio líquido pela técnica Liqui-PREPTM se equivalem, embora sejam mais insatisfatórias na técnica citológica Liqui-PREP®.

ABSTRACT Introduction: Various preparations can be used in diagnostic cytology, including conventional smears (CS), liquid-based preparations (LBP) and cell block (CB). Objective: The aim of this study is to evaluate the quality of CB preparations in addition to conventional cytological specimens in cases of fine-needle aspiration biopsy (FNAB) of thyroid nodules in diagnostic routine. Method: One hundred and six consecutive cases of FNAB routine thyroid nodules were independently evaluated by two cytopathologists (Obs1 and Obs2) on the cellularity of SM, LBP and CB. Results: The cellularity was rich/moderate in 56 (52.8%) CBs for both observers. LBP showed rich/moderate cellularity in 86 (81.1%) cases for Obs1 and 91 (85.8%) for Obs2; among these cases, CB showed the same cellularity in 52/86 (60.4%) cases for Obs1 and 54/91 (59.3%) for Obs2. SM showed rich/moderate cellularity in 86 (81.1%) cases for Obs1 and 87 (82%) for Obs2; among these cases, CB showed the same cellularity in 48/86 (55.8%) cases for Obs1 and 54/87 (62%) for Obs2. CB cellularity was higher than that in LBP in only five cases for Obs1 and three for Obs2. LBP was assessed as low/absent in only five (4.7%) and six (5.6%) cases for Obs1 and Obs2, respectively. Conclusion: CB can be routinely used as additional specimen in material obtained from thyroid nodules FNAB, without adversely affecting LBP specimens, enabling the conduction of further immunohistochemical and molecular studies.


RESUMO Introdução: Vários preparados podem ser utilizados na citologia diagnóstica, como esfregaços (SM), preparados do tipo meio líquido (ML) e emblocado em parafina ou cell block (CB). Objetivo: Avaliar a qualidade do CB, além dos espécimes citológicos convencionais, em casos de biópsia aspirativa por agulha fina (BAAF) de nódulos de tireoide na rotina diagnóstica. Método: Cento e seis casos consecutivos de BAAF de nódulos de tireoide foram avaliados independentemente por dois citopatologistas (Obs1 e Obs2) quanto à celularidade dos preparados de SM, ML e CB. Resultados: A celularidade foi rica/moderada em 56 (52,8%) CB para ambos observadores. ML mostrou celularidade rica/moderada em 86 (81,1%) casos para o Obs1 e 91 (85,8%) para o Obs2; desses casos, CB mostrou a mesma celularidade em 52/86 (60,4%) casos para o Obs1 e 54/91 (59,3%) para o Obs2. SM mostrou celularidade rica/moderada em 86 (81,1%) casos para o Obs1 e 87 (82%) para o Obs2; desses casos, CB apresentou a mesma celularidade em 48/86 (55,8%) casos para o Obs1 e 54/87 (62%) para o Obs2. A celularidade do CB foi maior do que a do ML em apenas cinco casos para o Obs1 e três para o Obs2. ML foi avaliado como escasso/ausente em apenas cinco (4,7%) e seis (5,6%) casos para os Obs1 e Obs2, respectivamente. Conclusão: CB pode ser utilizado rotineiramente como espécime adicional no material obtido de BAAF de nódulos de tireoide, sem prejuízo dos espécimes de meio líquido, o que possibilita a realização de estudo complementar, principalmente imuno-histoquímico e molecular.

Objective@#To describe the cytologic features of adenoid cystic carcinoma (ADCC) of salivary glands, and to identify distinguishing cytologic features of ADCC and basal cell adenoma (BCA).@*Methods@#A retrospective review of cytology smears of 30 cases of ADCC and 12 cases of BCA of salivary glands were performed. All cases were collected from Beijing Tongren Hospital, Capital Medical University from January 2010 to January 2017. Except for 2 aspirate smears of ADCC, all were touch imprint smears. All cases had further histological confirmation.@*Results@#Neoplastic ductal cells of ADCC were arranged in three-dimensional clusters, sheets and singles. Hyaline globules were found in most cases (20/30, 66.7%). The nuclei were round to oval, showing varying degrees of nuclear atypia. These included (1) the nuclei were hyperchromatic, demonstrating coarse or slightly coarse, irregularly distributed chromatin; (2) the nuclei were slightly large and vary in size; (3) appearance of the nuclei had a different degree of irregularity (often mild). Nucleoli were common seen (21/30, 70.0%), and were prominent in some cases. Mitosis and necrosis were rare. Cytologically, BCA showed cell arrangements and nuclear features overlapped with those of ADCC. The cytologic difference between these two tumors included: (1) the tumor cells presented rarely in singles; (2) hyaline globules were very uncommon (1/12) in BCA; (3) nuclei of BCA were hypochromatic or slightly hyperchromatic, homogeneous and uniform in appearance and size, overall without nuclear atypia and they were smaller and slender then those of ADCC and (4) individual cells of BCA showed relatively abundant cytoplasm.@*Conclusions@#The cytologic features of ADCC and BCA both overlap and different from each other. Most cases can be diagnosed by cytologic examination. The presence of hyaline globules is an important diagnostic clue of ADCC, although not pathognomonic. Nuclear atypia of neoplastic ductal cells is an essential cytological feature in the diagnosis of ADCC, and is the most reliable point for differential diagnosis of ADCC and BCA.

Objective@#To investigate the human papillomavirus (HPV) positive rate and its usefulness in predicting CIN2+ in women with atypical squamous cells of undetermined significance (ASC-US) cervical cytology.@*Methods@#A pooled analysis was conducted using published data of hospital classification, HPV positive rate and histopathologic diagnosis in ASC-US population during 2005 to 2017 from 104 studies which enrolled 28 923 ASC-US samples.@*Results@#The overall HPV positive rate was 52.09% (range from 12.06% to 88.68%). The HPV positive rate in 79 tertiary hospitals of 21 244 cases was 52.46%, slightly higher than the 50.87% in 22 second-class hospitals of 6 925 cases. There was no significant difference between specialized hospitals and general hospitals. In addition, the positive rate of HC2 conducted in 66 hospitals with 19 791 cases was 53.13%, which was slightly higher than 51.10% of reverse hybridization from 24 hospitals with 6 338 cases. In 73 studies of 18 163 cases with histological diagnosis, the sensitivity of HPV for detecting CIN2+ was 90.16% (95%CI: 88.91% to 91.28%), specificity was 53.08% (95%CI: 53.02% to 54.57%), positive predictive value was 23.24% and negative predictive value was 97.24%.@*Conclusion@#HPV detection is clinically validated for ASC-US triage, but there is a wide variation of HPV positive rate in population of cervical cytological diagnosis as ASC-US in China, suggesting different diagnostic level between regions and hospitals and further improvement is needed.

Objective To analyze the application of the HR -HPV,TCT combined with digital colposcopy cervical biopsy in diagnosis of CIN and cervical carcinoma.Methods From 2014 to 2016,250 cases diagnosed as ASCUS and above the level of disease were selected in Shanxi Cancer Hospital ,including 148 cases of ASCUS,68 cases of LSIL,27 cases of HSIL and 7 cases of SCC.The HR-HPV copy number was detected by using PCR fluorescence measurement,the Reid colposcopy scores were measured by using digital colposcopy ,and the cervical histopathological diagnosis was regarded as a measurement rule.Results The diagnostic rate of simple use of TCT for the diagnosis of CIN and above lesions was 41.20%(103/250), HR -HPV & TCT in diagnosis of CIN and above lesions was 58.78%(77/131),TCT & digital colposcopy for the diagnosis of CIN and above lesions was 67.42%(89/132), which were significantly increased (χ2=35.87,P=0.00;χ2=59.01,P=0.00).The diagnostic rate of HR-HPV&TCT combined with electronic colposcopy in diagnosis of lesions above CIN was 79.82%( 91/114 ), which was significantly higher than that of HR -HPV & TCT[58.78%(77/131)],and the diagnostic rate of TCT & digital colposcopy was 67.42%(89/132)(χ2=15.54,P=0.01;χ2=8.05,P=0.02).The incidence of CIN and cervical cancer were closely related with HR -HPV infection and the rate of HR-HPV infection showed an increasing trend with the aggravating degree of disease (the HR-HPV infection rate of ASCUS was 39.86%,the HR-HPV infection rate of LSIL was 61.76%,the HR-HPV infection rate of HSIL was 88.89%,while the HR-HPV infection rate of SCC was 100.00%).Conclusion HR-HPV,TCT combined with digital colposcopy can effectively improve the diagnosis rate of CIN and above the level of disease.At the same time,with the deterioration of the disease ,HR-HPV infection rate increases significantly ,which can provide diagnostic information and significance in the clinical diagnosis of CIN and cervical cancer.

ABSTRACT

Objective To explore the diagnostic value of cytologic examination of cerebrospinal fluid (CSF) combined with cell immunochemical staining examination in the patients with meningeal carcinomatosis in the clinical track.Methods CSF was centrifuged and precipitated using the StatSpin Cytofuge 12 centrifuge. It was dyed by the May-Grunwald-Giemsa Staining (MGG) method and cell immunochemical staining (S-P method). Clinical data of 16 cases with the diagnosis of meningeal carcinomatosis were retrospectively analyzed. Results Sixteen patients diagnosed meningeal carcinomatosis prior to imaging findings in 11 of 16 patients were diagnosed meningeal carcinomatosis through the cerebrospinal fluid cytology combined with cell immunochemical staining prior to imaging findings. CSF cytology observed the cancer cell pleomorphism. Cell immunochemical staining examination clearly distinguished classification organic source of meningeal carcinomatosis. Conclusions CSF cytology combined with cell immunochemical staining examination is one of the important means in tracking intracranial metastatic tumor in clinic.
